Admin Test Med Reminder

Send a one-off medication reminder for testing the adherence loop. Creates a same-day reminder row (last_sent_date=today, expires today) so a 'reply 1/2' or button tap logs a dose. Returns whether the number is an onboarded patient (required for the reply to be attributed).
