MCP connect

Start an OAuth consent for an MCP server URL before or without a server row (connect by URL). Requires server create permission in organizationId, or server update permission on the external server serverId (Settings update mode; its organization is used). After consent, create the server with pendingCredentialAttemptId (POST /servers) or bind it in an update (PATCH /servers/{id}).
