Add or increment a cart line item

Upserts on (product_id, variant_id, tender) — same triple = quantity increments rather than creating a duplicate row. Enforces stock (skipped when inventory_policy='continue' or track_quantity=false) and, in credits-mode, a wallet-balance gate.
