Published data-quality incidents (BASIC)

Every data-quality incident we have published, oldest window first. This is the register to reconcile against when a published value changed after you read it: each record states the window it applies to, the rule before and after, what the defect was, which surfaces it touched, how to tell an affected row from a clean one, and whether corrections and a per-match list are available. Nothing here is inferred at read time. The register is part of a release, so a record appears only when we have decided what it says, and it does not change between releases.
