MCP tool definition SHA-256 manifest

Maps MCP tool identifiers (name in each GET /mcp/lithtrix-*.json body) to SHA-256 of the verbatim JSON bytes returned by those routes. Responses use 3600s Cache-Control and a stable document ETag; send If-None-Match to receive 304 when unchanged.
