Three more public records for one NPI of a provider check: its Medicare contractor, its state Medicaid enrollment and the SAM.gov exclusion list.

Send the email that sent the list and one NPI of that check, within 24 hours of sending it. Returns one line per record, each with its source link and read time and a state: read (the source answered; a real negative is said in the summary), needs_person (a person must look; reason says why) or not_covered (not read automatically; reason says why). Medicaid has one line per state (the practice state, the states of the provider's licenses, then the mailing state; at most four). A SAM.gov listing by NPI has facts.excluded true; a possible match by name is needs_person. These records are not part of the check's rows or its emailed table. Reading takes up to about 20 seconds. Limits: 30 requests per address per 10 minutes; up to 25 NPIs and 50 requests per check; and an overall limit across callers. Past a limit the answer is 429.
