Provider - Activate

Reactivates a provider that was soft-deactivated via DELETE — sets active=true. The inverse of DELETE /providers/{id}. Kept as a dedicated action (not PATCH) so the deactivate/reactivate pair stays out of config-edit audit noise. Takes ~2 minutes to affect live search (cache refresh cadence). Note: this does NOT touch paused — a provider that was paused before deactivation comes back still paused; resume it separately if needed.
