Refresh

Exchanges a refresh_token for a new access/refresh token pair. 401 invalid_refresh_token if the token is unknown or was already reused (reuse of a rotated-out token revokes the whole family — a theft signal), 409 refresh_raced when a concurrent refresh in the same family already rotated it (benign — retry with the client's latest stored token, do not treat as a logout).
