Claim a pending work item

Atomically transitions the run to claimed and returns the claimToken that fences all subsequent calls. Heartbeat within leaseSeconds or the claim is reaped as abandoned. Part of the document pipeline — answers 503 feature_disabled until DOCUMENT_INGEST_ENABLED=1.

Required scope: indexer:write.
