Replace multiple orders using cancel-all-then-create-all ordering.

Every cancellation is submitted before any replacement order is placed. Results remain in request order. If a cancellation fails because the old order was already filled or cannot be found, its replacement is skipped and the result reports the cancellation failure. The batch is not atomic: successful cancellations may complete before their replacements are accepted.
