Remove an issuer configuration.

Refused with 409 while live bindings still name it. A cascade would deprovision N workloads under an operation whose name says "configuration", and an orphaning delete would leave bindings that silently stop authenticating. The operator revokes the bindings first, which is the same act stated honestly.
