The options for every finite-valued policy condition

Keyed by value set, so a check's attribute_value_set looks its options up directly. Restricted-industry options come from the selected account's settings, and crm_decline_reason lists the codes a check's decline_code can take, named from the account's CRM integration. Admins may select an account; customers always read their own.
