Create Interview Attempt By Code
Record an attempt identified by the member's CODE rather than their id.
The path a candidate flow uses, where the code is the only thing on hand. An EXPIRED code is a 410, not a 404 - the distinction matters, because a 410 means "this was real and the window closed" and is worth re-inviting for, while a 404 means the code never existed.
