Search records with the hosted MCP schema

REST bridge for the fonteum_search_providers MCP tool and source-only @fonteum/mcp 0.5.0 contract. Requires a healthcare vertical and state; it returns the same dated source-context envelope as hosted MCP. This is distinct from the semantic SSE /search endpoint.
