POST /api/mcp - Handle JSON-RPC requests.

Accepts JSON-RPC requests and returns either:

  • application/json for simple responses
  • text/event-stream for streaming responses (not implemented yet)

The Mcp-Session-Id header is assigned on initialize and required for subsequent requests.
