Create an outside-hauler driver profile

Creates a user in the configured outside-hauler company, provisions Auth0 SMS login (no welcome email), and creates a driver profile with vendor_id and branch links. email_address is optional; when omitted or blank, a placeholder {digits}@noemail.com is generated from the cell phone number.
