Refresh access token using refresh token.
Accepts refresh token from:
- Request body (preferred - frontend explicitly sends refresh_token)
- HTTP-only cookie (secure alternative)
- Authorization header (legacy - but NOTE: apiClient sends ACCESS token here!)
Returns new access token and optionally rotates refresh token.
