Create Child Workspace

Create a customer workspace. Returns 201 if new, 200 if external_id already existed.

Nesting is one level: the customer workspace is created against the caller's own workspace, and a header cannot be used to create a second-level child. Requires Idempotency-Key header for retry safety.
