Disconnect Connection

Revoke an integration connection.

Best-effort provider revocation (SAVEPOINT-isolated) + local token scrub + mark as revoked. Idempotent: disconnecting an already-revoked connection returns success.

Returns 200 with {"status": "revoked"} on success (both newly revoked and already-inactive cases). Returns 404 if the connection does not exist or belongs to a different workspace (confused-deputy guard).
