Recover access after auth token loss

Recovers an agent whose auth_token was lost, using the recovery_key issued at registration (or last rotate/recover). No authentication required: possession of agent_id + recovery_key is the proof. On success both auth_token and recovery_key are replaced (old values immediately invalid). Strictly rate-limited.
