Fetch one tenant-bound Decision Dossier

Returns the persisted Decision Dossier for an authenticated tenant. An ENFORCEMENT-mode ESCALATE dossier carries authority_requirement only when a matched policy rule explicitly defines Presence authority and the evaluation supplied a valid canonical context.action_context.
