Delete avatar

Delete the avatar for the authenticated user. Returns the updated user with avatar removed. Also deletes the physical file from storage and the document record from database.

Required entitlement: regulated.my-profile.canModifyMyAvatar
Entitles the regulated user to upload and delete their own avatar
