Get index health signals

Returns read-only index-health signals for the instance database from PostgreSQL catalogs: tables with heavy sequential scans, unused indexes, duplicate indexes, and Odoo many2one fields lacking a leading btree index. Signals are raw and intentionally conservative (unique/primary indexes are excluded from unused-index signals); interpretation is left to the caller. Returns HTTP 409 with a reason if the database is not reachable for inspection.
