Create (or regenerate) the API key

Returns the full key once (ck_ + 64 hex). Use it as Authorization: Bearer ck_… on every /api/* route and on POST /mcp. Requires a plan with api_mcp (Starter and above) → 403 otherwise. Regenerating revokes the previous key.
