Records a sign-off. 409 `INSTRUCTIONS_CHANGED` when the instructions were edited after the screen showed them (the request carries a fingerprint of the text shown), and 409 `DISCLAIMERS_CHANGED` with the live blocks when the client was shown a stale set of notices. The document row is locked while checking and writing, so an edit can't land between the check and the sign-off.

SHA-256 (hex) of the instructions as shown to the client: the text with CRLF as LF and edge whitespace trimmed. The sign-off is refused if the stored instructions differ, so a signature never covers text the client didn't see.
