Apply Bank Details Change

Approve/apply a staged bank change created from a bank_details_form submission.

  • Finds the staged bank change row for the given form_request_id
  • Applies bank details to the underlying entity (participants or pm_providers)
  • Marks the bank change row as approved (and sets decided_at/approved_by)
