Revoke Grant
Revoke every access + refresh token issued to client_id for the authenticated account. Returns 200 with the count of tokens flipped (access_revoked + refresh_revoked).
404 GRANT_NOT_FOUND when no active grant exists for that (account, client). Idempotent calls hit this branch -- the second Revoke click on a row that's already gone returns 404, which the UI handles by just refreshing the list.
