Remote MCP transport proxy

Streamable-HTTP proxy to a registered upstream MCP server. The body is a JSON-RPC 2.0 message; AxioRank inspects, audits, and polices tools/call before forwarding. Authenticated with a per-server URL token (or the governing agent's Bearer key), NOT the standard gateway scopes.
