Reject a pending action

Reject a suspended HITL pending action with a bounded server-validated reason. Same act:approve scope gate as /approve (held back pending legal review). NO government call is ever made on this path; REJECTED is terminal.

Bounded server-validated rejection reason. Printable ASCII + Norwegian letters (ÆØÅæøå) only; 1-512 chars. Stored on pending_actions.reject_reason for the compliance trail. The pattern here mirrors the Zod check the route runs BEFORE the DB CHECK fires.

REJECTED — terminal.
