Stream this conversation's outbox as Server-Sent Events.

The connection is held open indefinitely (heartbeat comments every 15s, a server-side cap after which the client reconnects and resumes from Last-Event-ID), so the database client is acquired once, here, and never inside the poll loop. It outlives the request, so it is minted a token of its own rather than reusing one that may expire mid-stream.
