Public rate-limit / quota tier table.

The per-tier throttling numbers (requests/min, burst, max concurrent /v1/plan, daily cap), how to authenticate for a higher tier, and the meaning of the X-RateLimit-* / Retry-After headers. Every /v1 response carries X-RateLimit-Limit, X-RateLimit-Remaining and X-RateLimit-Reset when enforcement is on; 429 and 503 add Retry-After.
