List Achievements

Every achievement for the authenticated user, unlocked and locked, in unlock order. Reconciles on read: anything newly earned is granted before the response is built, so the first dashboard load after an achievement is earned already shows it. Grants are append-only and never revoked, so unlocked never returns to false once true.
