Chat Completions

OpenAI-compatible chat endpoint that bridges bot answers to the orchestrator.

First request: bot connects, we return the first question (no inbox push). Subsequent: bot's answer goes to inbox, next question returned from outbox. On RESET: ack the adapter, clear history, wait for next question. On COMPLETE: final message with report link returned.
