Single JSON-RPC entry point for initialize / tools/list / tools/call. Auth = the existing app_token Bearer, tenant-scoped via current_tenant_ctx (deny-by-default). A malformed body returns a proper JSON-RPC parse/invalid-request error; a tool failure returns isError, never a 500.
