Resolve Code

Canonical QR resolver — supports two entry types:

entry_type='service_point' FD-T / FD-C codes (table, counter, etc.) entry_type='menu' FD-M codes (restaurant/menu QR, no table context)

Both types produce the same canonical response shape; the frontend decides presentation based on entry_type. Legacy LM* codes were migrated to FD-T/FD-C by migration 136 and still resolve via service_points.
