Find CPT codes matching procedure text — deterministic, no LLM.

Hybrid full-text + trigram retrieval over the CPT corpus, with surgeon-shorthand expansion ("lap chole" → laparoscopic cholecystectomy), axis-aware rescoring, and family expansion so with/without and approach variants are visible rather than pruned.

Every returned code is verified to exist in the CPT code set for the year. This is the same retriever the chart→CPT pipeline uses, and it is the only component that can produce a CPT code at all.

Scores are RETRIEVAL scores, not confidence. They say how well the query's wording matched a code's phrasings — nothing about whether the code is billable, correctly modified, or supported by documentation. Use /cpt/validate for that.

⚠ corpus_ready: false means the answer is empty because the corpus is unbuilt, not because nothing matched. Those are different facts and the response distinguishes them.

⚠ Descriptors are not returned: they are AMA-licensed, and this response is unmetered reference output. Use GET /cpt/codes/{code}, which gates descriptor text on AMA_CPT_LICENSE_KEY.
