Flush Metered Usage

Report all unreported metered usage events for the calling key.

Routes to the correct billing provider based on the org's billing_provider field (Stripe / AWS Marketplace / Azure Marketplace). Idempotent: events already reported (reported_to_stripe_at IS NOT NULL) are skipped, and each provider-specific path uses event id as the idempotency key so partial failures + retries do not double-charge.
