Logout

Log out — clear cookies, invalidate the refresh token AND the live access token.

Logout is account-wide: user_accounts holds exactly one refresh token, so there is no per-device session to end separately. Before 2026-09-24 the access JWT stayed valid for up to 15 minutes after logout; end_sessions now sets the revocation cutoff so it dies with the refresh token.
