Finish a question paper upload

Step 2 of 2. Call once every URL from initiate-upload has been uploaded to. Creates the question paper and queues it for extraction.

Extraction is asynchronous — the paper comes back QUEUED. Poll GET /v1/question-papers/{id}: it becomes GENERATED when the questions are ready, or FAILED if the PDF could not be read. Name, marks, duration and language are all read from the PDF.

Safe to retry: calling it twice with the same upload_ref returns the paper already created rather than creating a second one.

Lifecycle of a question paper.

Mirrors the internal QuestionPaperStatus one-for-one — a paper being generated moves QUEUED -> PROCESSING -> GENERATED, and APPROVED once a teacher signs it off.
