Vapi custom LLM endpoint — OpenAI-compatible SSE streaming.

Vapi expects a streaming SSE response (Server-Sent Events) so it can
start TTS on the first sentence while the rest is still generating.
Without streaming, Vapi times out and sends 'hang' after ~5s.
