`POST /v1/hospice/adr/respond`

Generate an ADR response for a hospice claim and persist it.

Loads ``merged_extraction`` from the DB (if configured) or the
in-memory store, re-scores hospice inference deterministically,
runs the generator, and appends the response to
``workflow_state["hospice_adr"]["responses"]`` so the artifact
survives server restarts and is retrievable for audit.

    - `claim_id` string, nullable: Provider's internal claim id (separate from Zicron claim id).

    - `medicare_id` string, nullable
    - `dates_of_service` string, nullable: e.g. '2023-08-15 through 2024-02-29'.

      - `finding_id` string, required: Short stable id, e.g. 'F1', 'F2'.

      - `cited_lcd_criterion` string, nullable: LCD criterion referenced by auditor, e.g. 'L33393 — EF ≤ 20%'.
      - `cited_regulation` string, nullable: CFR / Medicare regulation reference, e.g. '42 CFR §418.22'.
    - `raw_text` string, nullable: Full pasted letter text; used as fallback when findings == [].
