SSE stream for real-time claim field updates.

Unlike the pipeline stream (/stream) which auto-closes on pipeline_complete,
this stream stays open indefinitely and only delivers field-level events
relevant to intake and multi-actor collaboration (e.g. intake_field_updated,
conversation_completed, claim_status_changed).
