Create a consent record for a claim.

A contact channel (sms, voice, email) is the claimant's contact consent (``conversations/consent.py``): a
person records it and says how it was obtained (``obtained_via``; 422 without), ``granted: false`` records
a withdrawal; refused (409) for texts or calls after the claimant texted STOP. It then counts for outreach.
