Link a phone number to a user account. Admin-only.

Optional: {"user_id": "uuid"} to link a different user (default: current user).

Phone number is normalized to E.164 via `normalize_phone` on input,
eliminating format-drift bugs between OTP linking, admin linking, and
the Sendblue inbound handler's lookup.
