Record a push ping for ``app_id`` and wake the push worker.

Unauthenticated by design (module docstring). The key of the rate
cap is the source IP: Caddy overwrites ``X-Forwarded-For``, and only
Caddy reaches this listener, so the value is the real peer. The cap
is DoS hygiene, not an authorization boundary.
