Approve on-demand TLS issuance for verified custom domains.

Deliberately unauthenticated and public: the only information it
leaks is a boolean "is this domain attached and verified?" — an
attacker learns nothing they couldn't learn by connecting to the
hostname over TLS. One indexed SELECT per call (``domain`` has a
unique index), so a probe flood costs one point lookup per call.
