`POST /apps/{app_id}/channels/{channel_id}/forward`

Allocate (or update) the channel's public raw-TCP endpoint.

Re-POSTing an already-exposed channel returns the same host/port with
``allow_cidrs`` replaced by the submitted list, and ``200`` instead of
``201`` — the endpoint address is stable for the life of the exposure
so tenants can hand it out.
