`POST /api/v1/sessions/{session_id}/external-decision`

Submit an external decision for a paused workflow.

Requires the ``sessions:external-decision`` scope (not included in default
scopes — must be explicitly granted to the API key).

The session must be in 'awaiting_external' status. The decision controls
how the workflow resumes:
- "continue": resume executing the remaining steps
- "fail": apply the gate step's on_fail action
- "reject": immediately reject the session
