Open a review case anchored to this identity.

Phase 4 of identity-centric refactor — cases are about *people*, so we
expose a direct ``POST /identities/{id}/cases`` path.  If ``session_id``
is supplied, the case is scoped to that session as well (useful when the
case was triggered by a specific attempt); otherwise the case is purely
identity-level.
