# List available Paperform forms

`GET /cohorts/{cohortId}/intake/forms`

## Request

- Base URL: `https://api.production.yourmedley.com`
- URL: `https://api.production.yourmedley.com/cohorts/{cohortId}/intake/forms`
- Auth: HTTP bearer

Returns a list of available forms from Paperform for selection.
If Paperform API is not configured, returns configured: false.

- `cohortId` string, uuid, required
